Brexpiprazole falls under SEROTONERGIC DRUGS — the group Black Seed interacts with.

Theoretically, combining serotonergic drugs with black seed might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders.
Animal research suggests that black seed can increase brain serotonin levels. In one case report, a 35-year-old man undergoing endoscopic surgery experienced immediate postoperative serotonin syndrome that was likely associated with the use of black seed oil 600 mg daily starting 4 days before surgery, and precipitated by the use of serotonergic pain medications, including fentanyl and oxycodone. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using black seed with serotonergic drugs.

Here's the idea with Rexulti (brexpiprazole) and black seed. Black seed may nudge serotonin levels up in the brain, at least based on animal work and one case report involving black seed oil with strong pain medicines. Brexpiprazole isn't a classic serotonin drug like an SSRI, but it does touch serotonin pathways, so the worry is that stacking the two could add up.

To be honest, this is a theoretical, low-evidence concern. Real-world serotonin syndrome from this pair is unlikely, but it helps to know the warning signs: agitation, fast heartbeat, sweating, shivering, muscle twitching, or confusion. A pinch of black seed as a kitchen spice is very different from concentrated daily supplement doses. Before starting a black seed supplement, just run it by your pharmacist or doctor.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
