Here's the thing to know about Glucovance (glyburide plus metformin): both of these medicines already work to lower your blood sugar. Black seed, mostly through its compound thymoquinone, also seems to nudge blood sugar downward. Stack them together and you could end up lower than you bargained for. That can show up as shakiness, sweating, a racing heart, dizziness, confusion, or feeling suddenly hungry.

This is more of a theoretical concern, backed by some lower quality studies and a lot of animal research, so I wouldn't panic. Food-amount use of black seed as a spice is unlikely to cause trouble, but concentrated supplement doses are the bigger question mark. If you want to try it, check with your pharmacist or doctor first and keep an eye on your sugar readings.

Glyburide, Metformin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Here is the gist for Glucovance, which combines glyburide and metformin. The glyburide half is partly cleared by a liver enzyme called CYP2C9. Black seed contains a compound called thymoquinone that, in a test-tube study, slowed that enzyme down. If that happened in your body, glyburide could build up a bit and push your blood sugar lower than expected. With glyburide, that means watch for shakiness, sweating, dizziness, hunger, or confusion, the usual signs of low blood sugar.

I want to be honest though: this is only a theoretical concern based on lab work, not anything proven in people, and black seed's real effect on this enzyme is still unclear. A pinch as a spice is unlikely to matter. If you are using concentrated black seed capsules or oil, just give your pharmacist or doctor a heads-up first.
