Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Black Seed interacts with.

Theoretically, taking black seed with antihypertensive drugs might increase the risk of hypotension.
Clinical research suggests that black seed powder and oil might reduce blood pressure by 2-3 mmHg. In animal research, black seed modestly reduces blood pressure and concomitant use of black seed and amlodipine (Norvasc) or metoprolol (Lopressor) increased the blood pressure lowering effects of these drugs.

Opsynvi combines two medicines (macitentan and tadalafil) that both work to relax blood vessels and lower pressure in the lungs. As a side effect, they can also lower your overall blood pressure. Black seed has its own mild blood pressure lowering effect, usually only about 2 to 3 mmHg in studies, so stacking the two together could nudge your pressure down a little more than expected.

What that might feel like is dizziness, lightheadedness, headache, or feeling faint when you stand up quickly. This is a moderate, mostly theoretical concern, and small food or spice amounts of black seed are unlikely to cause trouble. Concentrated supplement doses are the bigger question mark. Before adding black seed, please check with your pharmacist or doctor.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Opsynvi combines two active medicines, macitentan and tadalafil, that your body partly clears using a liver enzyme called CYP2C9. The concern here is that a compound in black seed (thymoquinone) might slow that enzyme down. If it does, these drugs could hang around a little longer and their effects could add up. In real life that might look like more low blood pressure, dizziness, headache, flushing, a stuffy nose, or swelling.

Here is the important part: this is theoretical. It comes from a lab study with a different drug, not from people taking black seed with Opsynvi, and its true effect on CYP2C9 is unclear. Food-amounts of black seed as a spice are unlikely to matter. Before starting a concentrated supplement, please check with your pharmacist or doctor.
