Treprostinil (IV) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Black Seed interacts with.

Theoretically, black seed may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that black seed extract can inhibit platelet aggregation and clotting, and increase bleeding time. In addition, decreased platelet counts have occurred in a human case report and in animal research.

Here's the thing to know about pairing black seed with intravenous treprostinil (Remodulin). Treprostinil itself isn't an anticoagulant, but it does relax blood vessels and naturally tends to make platelets less sticky, so it can add to bleeding risk on its own. Black seed, at least in lab and animal studies, seems to do something similar, slowing clotting and even lowering platelet counts in a few reports.

Stack those two effects together and the worry is easier bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding. This is still theoretical and based mostly on test-tube and animal work, not strong human proof. Food-amount black seed as a spice is unlikely to matter, but concentrated supplements are less predictable. Since you're on IV treprostinil, please check with your doctor or pharmacist before adding it.

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.

It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
