Ryzodeg combines two insulins, a fast-acting one (insulin aspart) and a long-acting one (insulin degludec), to help keep your blood sugar in a healthy range. The concern here is that blue cohosh may push blood sugar up, which could work against your insulin and make it harder to reach your targets. In practice that might show up as higher readings on your meter, more thirst, or more frequent urination.
I'll be honest with you: this interaction is theoretical, based on how the herb behaves in lab settings, not on solid human studies. Even so, blue cohosh has real heart and pregnancy safety worries on its own, so it's an herb I'd steer most people away from. Please loop in your doctor or pharmacist before mixing it with your insulin, and don't change your insulin dose on your own.
