Constituents in blue cohosh might increase blood pressure by causing coronary vasoconstriction. Theoretically, concomitant use might decrease the effectiveness of drugs used for angina and high blood pressure; use with caution. Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), Amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.

Here's the concern with pairing blue cohosh and losartan (Cozaar). Some compounds in this herb may tighten blood vessels and nudge blood pressure upward. Since losartan works to relax your vessels and lower blood pressure, the two could end up working against each other. In plain terms, the herb might blunt some of the benefit you're getting from your medication, leaving your numbers higher than they should be.

I'll be honest, this is a theoretical worry based on how blue cohosh behaves, not on solid human studies, so I don't want to alarm you. Still, blue cohosh carries real safety issues of its own, especially for the heart and in pregnancy. Please check with your pharmacist or doctor before combining them.

Blue cohosh is a traditional herb historically used by Native Americans and midwives to bring on labor or menstruation, but it carries serious safety concerns, especially for the heart and during pregnancy. There is very little reliable human evidence that it works, and reports of harm—including serious problems in newborns—make it an herb most people should avoid without close professional supervision.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
