Bog Labrador Tea and Modafinil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bog Labrador Tea interacts with.

In vitro research shows that aqueous and ethanolic extracts of Bog Labrador tea leaves can increase the concentration of cytochrome P450 3A4 (CYP3A4) enzymes. Extracts obtained from different geographical regions in Quebec resulted in differing potency of CYP3A4 induction. Bog Labrador tea might increase the metabolism of CYP3A4 substrates.
Drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), amiodarone (Cordarone), ticagrelor (Brilinta), triazolam (Halcion), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Modafinil (Provigil). Your body uses a liver enzyme called CYP3A4 to help clear modafinil out of your system. In lab dish studies, extracts of Bog Labrador tea seemed to speed up (induce) that enzyme. If that happened in a real person, your body might break modafinil down a little faster, which could leave you with less wakefulness and more daytime sleepiness than you'd expect, especially if you rely on it for narcolepsy or shift work.

I want to be honest about how thin this evidence is. This comes only from test-tube and animal research, not from people, and the strength even varied by where the plant was grown. So it's more of a theoretical concern than something we've actually seen cause trouble. Still, if you'd like to try the tea, run it past your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Bog Labrador Tea

What is Bog Labrador Tea?

Bog Labrador Tea
  • Coughs and respiratory complaints
  • Digestive upset and diarrhea
  • Anxiety and stress
  • As a warm beverage
  • Aches and pains (traditional use)

Bog Labrador tea is a wetland shrub whose leaves have long been brewed as a traditional beverage and folk remedy, especially among Indigenous peoples of North America. There is very little human research to confirm any health benefit, and the plant is closely related to species that contain toxic compounds, so it should be used cautiously and in moderation. Pregnant or breastfeeding people should avoid it, and anyone considering it should talk with a pharmacist or doctor first.

About Modafinil

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
