Nirogacestat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bog Labrador Tea interacts with.
Drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), amiodarone (Cordarone), ticagrelor (Brilinta), triazolam (Halcion), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: Nirogacestat (Ogsiveo) is cleared from your body mainly by a liver enzyme called CYP3A4. Lab studies suggest Bog Labrador tea may rev up (induce) that enzyme, which could make your body break down nirogacestat faster. In plain terms, that might lower the drug level in your blood and make it work less well against your desmoid tumor. Nirogacestat is not a prodrug, so faster clearance means less benefit, not more.
I want to be honest about the evidence though. This is based on test-tube and extract studies, not people, and the strength varied by where the plant was grown. So it's a possible concern, not a proven one. Since nirogacestat matters for controlling your tumor, please run this by your oncology team or pharmacist before mixing the two.
