Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Broccoli Sprout interacts with.

Theoretically, broccoli sprout might reduce levels and effects of drugs metabolized by CYP1A2.
Animal research shows that broccoli sprout extract can increase CYP1A2 gene expression. Also, pharmacokinetic research in humans shows that eating 500 grams of fresh broccoli daily for 6-12 days can increase CYP1A2 activity by 10% to 200%. Induction of CYP1A2 activity by broccoli is attributed to glucosinolate constituents, which are also found in broccoli sprout.

Here's the gist for Esbriet (pirfenidone): your liver clears it mainly through an enzyme called CYP1A2. Broccoli sprouts contain compounds that may gently rev up, or induce, that same enzyme. If CYP1A2 speeds up, your body could clear pirfenidone a bit faster, which in theory might lower its blood levels and make it work less well against the lung scarring it's meant to slow.

I want to keep this in perspective, though. This is theoretical, based on pharmacology and a study using large daily amounts of broccoli, not concentrated sprout supplements with pirfenidone. Normal food portions are unlikely to cause trouble. Pirfenidone is a finished drug, not a prodrug, so the direction here is reduced effect. Before starting a concentrated supplement, just check with your pharmacist or doctor.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
