Why this applies to Mexiletine
Mexiletine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Broccoli Sprout interacts with.
Animal research shows that broccoli sprout extract can increase CYP1A2 gene expression. Also, pharmacokinetic research in humans shows that eating 500 grams of fresh broccoli daily for 6-12 days can increase CYP1A2 activity by 10% to 200%. Induction of CYP1A2 activity by broccoli is attributed to glucosinolate constituents, which are also found in broccoli sprout.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears mexiletine mainly through a liver enzyme called CYP1A2. Broccoli sprouts contain compounds that may gently speed that enzyme up. If that enzyme works faster, it could clear mexiletine a little quicker, which in theory might lower its blood levels and make it slightly less effective at keeping your heart rhythm steady. Mexiletine is an active drug (not a prodrug), so faster clearance means weaker effect, not stronger.
Now, don't panic. This is rated minor and is only theoretical, based on how the enzyme behaves. Everyday culinary amounts of broccoli sprouts on a salad are very unlikely to matter. The bigger question mark is concentrated sprout supplements taken daily. Since mexiletine controls a serious heart rhythm, just run any supplement plan past your doctor or pharmacist first.

What Is Mexiletine Used For?
Brand name: Mexitil
Mexiletine Hydrochloride capsules are indicated for the treatment of documented, life-threatening ventricular arrhythmias — for example, sustained ventricular tachycardia (a dangerously fast heart rhythm originating in the lower chambers). Because mexiletine itself can trigger arrhythmias, it is not recommended for minor or symptom-free irregular heartbeats.
Treatment is started in a hospital setting so heart rhythm can be monitored closely. Antiarrhythmic drugs, including mexiletine, have not been shown to improve overall survival in patients with ventricular arrhythmias.
