Fluconazole falls under HEPATOTOXIC DRUGS — the group Buchu interacts with.

Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)

Here's the thing to know about pairing buchu with fluconazole: this isn't about one changing the other's blood levels. Instead, it's about a shared burden on your liver. Fluconazole, like many antifungals, can occasionally stress the liver, and buchu leaf contains a compound called pulegone that may also be hard on the liver, especially in strong supplement doses or if the essential oil is swallowed. Stacking two liver stressors together is the worry here.

I'll be honest, this concern is theoretical and based on how these things work, not on real cases. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. A casual culinary amount is far less concerning than concentrated capsules or oil. Please check with your pharmacist or doctor before combining them.

Buchu is a fragrant South African shrub whose leaves are traditionally used for urinary and bladder complaints and as a diuretic. Solid human studies are lacking, so its benefits are largely unproven, and it should be avoided in pregnancy and breastfeeding. About Fluconazole

What Is Fluconazole Used For?

Brand names: Diflucan, Diflucan Injection

Fluconazole treats infections caused by Candida and Cryptococcus fungi. Diflucan tablets and oral fluconazole tablets and suspension are approved to treat vaginal yeast infections (vaginal candidiasis), thrush in the mouth or throat (oropharyngeal candidiasis), and yeast infections of the esophagus. All forms — tablets, oral suspension, and injection — are used for cryptococcal meningitis (a serious fungal brain infection). In smaller studies, fluconazole has also shown effectiveness against Candida urinary tract infections, peritonitis (infection in the abdomen), and serious systemic (bloodstream and organ) Candida infections.

Fluconazole is also used to prevent candidiasis in people undergoing bone marrow transplants who receive chemotherapy or radiation therapy. Note that vaginal candidiasis is an approved use for the tablets and Diflucan oral products, but not for the injection or oral suspension alone.
