Bempedoic Acid,Ezetimibe falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Caffeic Acid interacts with.

Theoretically, caffeic acid might increase the levels and clinical effects of OAT3 substrates.
In vitro, caffeic acid inhibits OAT3. This drug transport protein is involved in renal tubular uptake of some drugs from the blood and then elimination in the urine. Inhibition of this transporters decreases renal elimination and increases drug levels in the body. Although caffeic acid inhibits OAT3 in vitro, the clinical significance of this in humans is not known.

Here's the gist with Nexlizet (bempedoic acid plus ezetimibe), your cholesterol-lowering combo. In test-tube studies, caffeic acid blocks a kidney transporter called OAT3 that helps your body clear certain drugs out through the urine. Bempedoic acid is partly handled by this kind of transporter, so in theory, caffeic acid could slow its removal and let levels nudge upward.

I want to be honest though: this is purely lab-dish data, not something proven in real people. If levels did rise, the most likely changes would be more of bempedoic acid's known effects, like muscle aches or a bump in uric acid (gout risk). The amounts in coffee and fruit almost certainly won't matter. If you're thinking about a concentrated caffeic acid supplement, just run it by your pharmacist or doctor first.
