Here's the simple version. Coffee charcoal acts like a sponge in your gut. Because of its porous, charcoal-like surface, it can grab onto medicines you swallow and hold onto them so your body absorbs less. If you take it at the same time as Nexlizet (bempedoic acid and ezetimibe), it may soak up some of the drug before it gets into your system, which could make your cholesterol medicine work less well over time.

Good news: this is a theoretical concern based on how charcoal behaves, not strong human studies, and it's easy to manage. The usual advice is to space them out by at least two hours. Neither ingredient here is a prodrug, so the worry is simply reduced absorption. Check the timing with your pharmacist or doctor to be safe.
