Here's the piece to focus on with this combination pill: the codeine part. Codeine is an opioid that already causes drowsiness, and in animal studies calamus acts as a mild central nervous system depressant (it appears to boost a calming brain chemical called GABA). Put those together and you may feel more sleepy, foggy, dizzy, or slowed down than you'd expect from the medicine alone.

I'll be honest, this is a theoretical concern based on pharmacology and animal data, not strong human studies, so I don't want to alarm you. Still, calamus is best avoided for safety reasons on its own, and stacking sedatives is never something I'd guess at. Please check with your pharmacist or doctor before combining them.

Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP2D6 enzyme.

This combination pill contains codeine, and codeine is a bit unusual. It's a prodrug, meaning your body has to switch it on using an enzyme called CYP2D6 to turn it into morphine, which is what actually relieves pain. Lab studies suggest calamus can block that enzyme. If it does, your body may make less morphine, so you could get weaker pain relief from the codeine, not stronger.

I want to be honest that this is only test-tube evidence, not proven in people, so the real-world effect is uncertain. Even so, calamus has its own safety concerns (it's banned from foods in the U.S.), so it's one I'd steer clear of. Please run it by your pharmacist or doctor before mixing the two.

This combination pill contains codeine, which is a bit different from most medicines. Codeine is a prodrug, meaning your body has to convert it into morphine before it does much. Some of that processing involves CYP3A4. Lab studies suggest calamus can block CYP3A4, but this is only test-tube research, not human data, so the real-world effect is uncertain.

In theory, blocking that enzyme could shift how codeine is broken down and let some of the other ingredients linger a little longer. With more sedation or grogginess possible, I'd play it safe. Honestly, calamus has bigger safety concerns of its own (it's banned from foods), so it's one I'd skip. Please check with your pharmacist or doctor before mixing them.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
