Here's the gist with Ziac (which combines bisoprolol, a beta-blocker, and hydrochlorothiazide, a water pill). Both ingredients are already working to lower your blood pressure and, in the case of bisoprolol, to slow your heart rate. Animal studies suggest calamus can also nudge the heart to beat slower and weaker, which may push your blood pressure down further. Stacked together, the concern is your numbers dropping too low.

If that happened, you might feel dizzy, lightheaded, tired, or faint, especially when standing up quickly. Keep in mind this is theoretical and based only on lab and animal research, not solid human data. Calamus also has its own safety worries, so please check with your pharmacist or doctor before using it.

Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP2D6 enzyme.

Ziac combines bisoprolol (a beta-blocker) with hydrochlorothiazide (a water pill). The bisoprolol part is partly cleared by a liver enzyme called CYP2D6. In lab studies, calamus appears to slow that enzyme down. The concern is that with the enzyme working slower, more bisoprolol could linger in your body, which might nudge its effects a bit higher than expected.

In practice, that could mean a slower heart rate, lower blood pressure, dizziness, or feeling unusually tired. I want to be honest though: this is only based on test-tube research, not real people, so we don't really know how big a deal it is. Calamus has its own safety worries too. Before pairing them, please check with your doctor or pharmacist.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
