Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.

Here's the basic idea with calamus and clomipramine. Your body relies on a liver enzyme called CYP2D6 to help break clomipramine down and clear it out. In lab studies, calamus extract appears to slow that enzyme down. If that holds true in people, clomipramine could build up higher than expected, which may mean more of its usual side effects, things like dry mouth, constipation, drowsiness, dizziness, a faster heartbeat, or trouble urinating.

I'll be honest though, this is only based on test-tube research, so it's more of a theoretical concern than a proven problem. Calamus also carries its own safety worries (the FDA banned it from foods), so it's worth a quick chat with your pharmacist or doctor before combining the two.

Here's the basic idea with calamus and your clomipramine (Anafranil). Your liver uses an enzyme called CYP3A4 to help break down this medication. Theoretically, concurrent use of anticholinergic drugs and calamus might decrease the effectiveness of the anticholinergic drug.

Here's the gist for Clomipramine (Anafranil). One of the ways this medicine works in the body is by having a mild drying, or anticholinergic, effect. Calamus seems to push in the opposite direction. In lab dish studies, it blocks an enzyme called acetylcholinesterase, which leaves more of the chemical acetylcholine hanging around. In theory, that could slightly counteract some of clomipramine's anticholinergic action.

I want to be honest with you: this is theoretical and based only on test-tube research, not real patients, so I wouldn't lose sleep over it. The bigger concern with calamus is its own safety (it contains beta-asarone, which the FDA banned from foods). Please don't start or stop anything on your own. Just run it by your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
