Fluphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Calamus interacts with.

Theoretically, taking calamus with drugs metabolized by CYP2D6 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP2D6 enzyme.

Here's the basic idea with these two. Your body relies on an enzyme called CYP2D6 to break down and clear fluphenazine. In lab studies, calamus appears to slow that same enzyme down. If that happened in real life, fluphenazine could build up in your system and hit harder than intended.

For a drug like fluphenazine, more in the bloodstream can mean more side effects, things like deep drowsiness, dizziness, low blood pressure, and stiff or restless muscle movements. I want to be honest though: this concern comes only from test-tube research, not human studies, so we can't say it definitely happens. On top of that, calamus carries its own safety worries (the FDA banned it from foods) and is generally best avoided. Please check with your doctor or pharmacist before combining them.

Here's the idea behind this pairing. Fluphenazine has some anticholinergic activity, meaning part of how it works involves dampening a brain chemical called acetylcholine. Calamus appears to do the opposite in lab dishes, blocking the enzyme that breaks acetylcholine down, which leaves more of it around. In theory, that could push back against fluphenazine and make it work a little less well, potentially letting symptoms like agitation or hallucinations creep back.

I want to be honest with you, though: this is based only on test-tube and animal research, not real patients, so it's quite speculative. The bigger concern is calamus itself. It contains beta-asarone, which the FDA banned from food over toxicity worries, so I'd steer clear of it and check with your doctor or pharmacist before adding any supplement.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

About Fluphenazine

What Is Fluphenazine Used For?

Brand names: Prolixin, Permitil, Fluphenazine

Fluphenazine hydrochloride tablets and oral solution are indicated for the management of psychotic disorders. Fluphenazine Hydrochloride Injection is also used for psychotic disorders and can bridge patients to oral therapy.

Fluphenazine Decanoate Injection is a long-acting form specifically intended for patients who need extended antipsychotic treatment — for example, those with chronic schizophrenia who benefit from a single injection that can last up to four weeks or longer. It is not approved for behavioral problems related to intellectual disability, and it is not approved for dementia-related psychosis in elderly patients.
