Here's the gist: losartan (Cozaar) already lowers your blood pressure by relaxing your blood vessels. The concern with calamus is that, in animal studies, it slowed the heart rate and weakened the force of each heartbeat, which can nudge blood pressure down too. Stack those two effects together and you could, in theory, end up with blood pressure that runs too low.

What that might feel like is dizziness, lightheadedness when you stand up, tiredness, or even fainting. To be clear, this is based only on lab and animal research, not solid human studies, so it's more of a caution than a proven problem. Calamus also carries its own safety baggage (the FDA banned it from foods). I'd skip it, but please check with your pharmacist or doctor before making any changes.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's something worth knowing about losartan (Cozaar) and calamus. Losartan is actually a prodrug, meaning your body has to convert it into its active form, and the enzyme CYP3A4 helps do part of that job. Calamus, in lab studies, appears to block CYP3A4. So in theory, calamus could slow that activation step and leave you with somewhat less of losartan's active form, which might mean weaker blood pressure control.

I'll be honest though: this is based only on test-tube research, not real people, so we can't say how much it actually matters day to day. Separate from that, calamus has its own safety concerns and is generally best avoided. If you're using or considering it, please check with your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
