Canadian Hemp and Mannitol

Here is the interaction we found between these two, with the evidence behind it.

Moderate

DIURETIC DRUGS

Why this applies to Mannitol

Mannitol falls under DIURETIC DRUGS — the group Canadian Hemp interacts with.

Canadian hemp contains cardiac glycosides. Theoretically, concomitant use of potassium depleting diuretics and Canadian hemp can increase the risk of cardiac glycoside toxicity due to potassium depletion. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing Canadian Hemp and Mannitol. Canadian Hemp naturally contains cardiac glycosides, the same kind of heart-acting compounds found in digitalis drugs. Mannitol is an osmotic diuretic, meaning it pulls extra fluid (and some electrolytes, including potassium) out through your urine. When potassium drops too low, cardiac glycosides become more likely to push the heart into trouble, things like an irregular or dangerously slow heartbeat, nausea, or confusion.

I want to be honest that this is a theoretical interaction based on how these substances work, not on strong human studies. That said, Canadian Hemp itself is toxic and not something I'd recommend at all. Please talk with your doctor or pharmacist before combining anything with Mannitol, and let them guide safer options.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Mannitol page

“American hellebore tops my worry list for anyone on mannitol (Osmitrol, Bronchitol), an osmotic diuretic used to draw fluid off the brain or, when inhaled, to clear mucus in cystic fibrosis. The plant's alkaloids drop blood pressure and slow the heart, a dangerous match for a drug that already shifts fluid and pressure quickly.”

About Canadian Hemp

What is Canadian Hemp?

  • Heart-related swelling (dropsy/edema)
  • Fluid retention
  • Constipation
  • Urinary problems
  • Traditional folk remedy uses

Canadian Hemp (Apocynum cannabinum) is a toxic North American plant historically used as a heart stimulant and diuretic, but it contains potent cardiac glycosides that can cause dangerous heart and poisoning effects. It is not a safe or recommended dietary supplement, and there is no reliable modern evidence supporting its use. Avoid using it and talk to a pharmacist or doctor about safer, proven options.
