Cassia Cinnamon and Felbamate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Felbamate

Felbamate falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about felbamate: it's a seizure medicine that already carries a known risk of liver injury, which is why doctors monitor liver tests while you take it. The worry with cassia cinnamon is that it naturally contains coumarin, and in large, concentrated supplement doses taken over a long time, coumarin can stress the liver too. Stacking two things that can tax the liver may add up.

To be fair, this is a theoretical concern based on how these compounds behave, not strong human proof. The cinnamon sprinkled on your oatmeal or toast is very unlikely to matter. It's the daily high-dose capsules that raise the question. Before adding a cinnamon supplement, please check with your pharmacist or doctor.

From our pharmacist’s overview on the Felbamate page

“Felbamate (Felbatol) is a seizure medicine usually reserved for epilepsy that other drugs have failed to control, in part because it can stress the liver on its own. That history is why its 172 supplement and herbal interactions deserve real vigilance, starting with the 14 rated major.”

About Cassia Cinnamon

What is Cassia Cinnamon?

Cassia Cinnamon
  • Blood sugar support
  • Digestive upset
  • Antioxidant support
  • Flavoring and culinary use
  • Antimicrobial

Cassia cinnamon is the common, inexpensive cinnamon used in cooking, and it is also taken as a supplement, most often for blood sugar support. The evidence for its health benefits is mixed and far from proven, and because cassia is naturally high in coumarin, taking large amounts regularly may stress the liver. Use food amounts freely, but talk with a pharmacist or doctor before using high-dose supplements.

About Felbamate

What Is Felbamate Used For?

Brand name: Felbatol

Felbamate is not a first-line seizure medication. It is used only when a person's epilepsy has not responded well enough to other treatments and the seizures are severe enough that the serious risks of this drug are considered acceptable.

In adults, felbamate can be used alone or alongside other antiepileptic drugs (AEDs) to treat partial seizures (seizures that start in one part of the brain), with or without spreading to become a full seizure. In children, felbamate is used as an add-on treatment for both partial and generalized seizures linked to Lennox-Gastaut syndrome, a severe form of childhood epilepsy. Felbatol is specifically labeled for these uses in both adults and children.
