Cassia Cinnamon and Glyburide, Metformin

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Cassia Cinnamon interacts with whole categories of medication. Glyburide, Metformin falls into 2 of those categories, so it can interact with Cassia Cinnamon in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

HEPATOTOXIC DRUGS

1 of 2

Why this applies to Glyburide, Metformin

Glyburide, Metformin falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with Glucovance (glyburide plus metformin) and cassia cinnamon. This isn't about cinnamon changing how your medicine works in the bloodstream. Instead, the worry is about your liver. Cassia cinnamon naturally contains coumarin, and in large, daily doses over a long stretch it may put a little extra stress on the liver in sensitive people. Both glyburide and metformin can occasionally affect the liver too, so stacking them theoretically adds up.

I want to be honest: this is a theoretical concern based on pharmacology, not proven in studies. The cinnamon you sprinkle on oatmeal is not the issue. High-dose supplements taken regularly are. One extra note: cinnamon may nudge blood sugar lower, so watch for shakiness or sweating. Chat with your pharmacist before starting a concentrated supplement.

ANTIDIABETES DRUGS

2 of 2

Glyburide, Metformin also falls under ANTIDIABETES DRUGS.

Theoretically, cassia cinnamon may have additive effects with antidiabetes drugs.
Cassia cinnamon may lower blood glucose levels, and have additive effects in patients treated with antidiabetic agents. Dose adjustments to diabetes medications might be necessary.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Glucovance pairs two blood sugar medicines: glyburide, which pushes your body to make more insulin, and metformin, which helps your body use insulin better. Cassia cinnamon may nudge your blood sugar down a little on its own. So when you take it alongside Glucovance, the two can add together and push your numbers lower than you planned.

This isn't a scary or strong interaction, and the evidence behind cinnamon's blood sugar effect is honestly pretty mixed. The bigger concern is the glyburide piece, since that one can cause low blood sugar (shakiness, sweating, dizziness, confusion). Sprinkling cinnamon on your oatmeal is no problem. If you're taking concentrated daily capsules, mention it to your pharmacist or doctor and keep an eye on your readings.

About Cassia Cinnamon

What is Cassia Cinnamon?

Cassia Cinnamon
  • Blood sugar support
  • Digestive upset
  • Antioxidant support
  • Flavoring and culinary use
  • Antimicrobial

Cassia cinnamon is the common, inexpensive cinnamon used in cooking, and it is also taken as a supplement, most often for blood sugar support. The evidence for its health benefits is mixed and far from proven, and because cassia is naturally high in coumarin, taking large amounts regularly may stress the liver. Use food amounts freely, but talk with a pharmacist or doctor before using high-dose supplements.
