Tocilizumab falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to know about combining cassia cinnamon with tocilizumab (Actemra): this isn't about the cinnamon changing how your body breaks down the drug. It's about both potentially being hard on the liver at the same time. Tocilizumab can occasionally raise liver enzymes on its own, which is why your doctor checks bloodwork while you're on it. Cassia cinnamon contains a compound called coumarin that, in large concentrated doses over time, may also stress the liver.

Just to be clear, this concern is theoretical and based on how these things work, not on strong proof. Sprinkling cinnamon on your oatmeal is no problem. The caution is really about high-dose cinnamon supplements taken daily for a long stretch. If you take those, please loop in your pharmacist or doctor so they can keep an eye on your liver labs.

What Is Tocilizumab Injection Used For?

Tocilizumab injection is used to treat several serious inflammatory and immune-related conditions. Actemra (tocilizumab) is indicated for moderately to severely active rheumatoid arthritis in adults who haven't responded well to other disease-modifying drugs, giant cell arteritis in adults, systemic sclerosis-associated interstitial lung disease (SSc-ILD) to slow lung function decline, polyarticular and systemic juvenile idiopathic arthritis in patients 2 years and older, and severe or life-threatening cytokine release syndrome (CRS) caused by CAR T-cell therapy in adults and children 2 and older.

Actemra is also approved for hospitalized adults and children aged 2 and older with COVID-19 who are on corticosteroids and need supplemental oxygen or mechanical breathing support. Not every brand or route is approved for every use — your prescriber will choose the right form for your specific condition.
