Here's the thing about products like Midol Max Strength (acetaminophen, caffeine, and pyrilamine): the caffeine in them acts as a mild diuretic, meaning it can make you lose a little extra fluid and potassium through your urine. Castor oil works the opposite end of things as a strong stimulant laxative, and if you use it often, it can also flush out potassium through the bowel.

The worry is that stacking the two together could lower your potassium more than either alone. Low potassium can leave you feeling weak, crampy, tired, or give you an irregular heartbeat. To be fair, this is a theoretical concern based on how these work, not something proven in studies, and an occasional dose is unlikely to cause trouble. If you use castor oil regularly, check in with your pharmacist or doctor first.
