Siponimod falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the gist for Mayzent (siponimod): your body partly relies on a liver enzyme called CYP3A4 to clear this medicine. Lab studies suggest chokeberry may slow that same enzyme down. In theory, if siponimod is broken down more slowly, more of it could linger in your system, which might mean a stronger effect and a higher chance of side effects like a slow heart rate, dizziness, headache, or raised blood pressure.

I want to be honest about the evidence here: it's very thin. This is based on test-tube work plus a single case report with a different drug, not solid human studies. Small amounts of chokeberry in food likely won't matter much, but concentrated supplements are a different story. Since siponimod dosing is already carefully tailored, please run any chokeberry supplement past your pharmacist or doctor first.

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).

It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
