These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Cocoa interacts with whole categories of medication. Ethinyl Estradiol, Norgestimate falls into 3 of those categories, so it can interact with Cocoa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS
1 of 3Why this applies to Ethinyl Estradiol, Norgestimate
Ethinyl Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Cocoa interacts with.
Cocoa contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from cocoa and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's a twist worth understanding: with cocoa and your birth control pill, it's actually the cocoa's caffeine that gets affected, not the hormones. The estrogen in your pill (ethinyl estradiol) can gently slow down a liver enzyme called CYP1A2, which is the same enzyme that clears caffeine from your body. So in theory, the caffeine in cocoa (and coffee, tea, or soda) might hang around a bit longer and feel a little stronger.
In real life, this could mean you notice more jitteriness, a faster heartbeat, trouble sleeping, or feeling wired if you have a lot of chocolate or caffeine. This is based on how the enzyme works, not strong studies, and a normal piece of chocolate is unlikely to cause trouble. If you feel more sensitive to caffeine, just ease back, and check with your pharmacist if you have questions.

ESTROGENS
2 of 3Ethinyl Estradiol, Norgestimate also falls under ESTROGENS.
Cocoa contains caffeine. Estrogen inhibits caffeine metabolism.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's a twist on how this one works. It isn't that cocoa changes your birth control. Instead, the estrogen in your pill (ethinyl estradiol) can slow down how your body clears caffeine, and cocoa happens to contain a bit of caffeine. So while you're on Ortho Tri-Cyclen or a similar pill, caffeine may stick around longer and hit a little harder than usual.
In real life that might mean feeling jittery, a racing heart, trouble sleeping, or an upset stomach if you have a lot of cocoa or other caffeine sources. The caffeine in a normal serving of cocoa or chocolate is small, so most people won't notice much. If you're sensitive to caffeine or use concentrated cocoa supplements, just go easy and check with your pharmacist.

CONTRACEPTIVE DRUGS
3 of 3Ethinyl Estradiol, Norgestimate also falls under CONTRACEPTIVE DRUGS.
Cocoa contains caffeine. Oral contraceptives decrease the rate of caffeine clearance by 40% to 65%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something interesting about your birth control pill (Ortho Tri-Cyclen, Sprintec, and similar brands). The estrogen in it actually slows down how fast your body clears caffeine, by anywhere from 40% to 65%. Since cocoa naturally contains a little caffeine, the caffeine you do get from chocolate or cocoa may stick around longer and feel a bit stronger than usual.
In real life, this usually just means you might notice more jitteriness, a racing heart, or trouble sleeping, especially if you're also drinking coffee or energy drinks on top of it. This is a minor interaction, and a normal square of chocolate isn't likely to be a problem. If you take concentrated cocoa supplements or feel wired, mention it to your pharmacist.

About Cocoa
What is Cocoa?
- Heart and blood vessel health
- High blood pressure
- High cholesterol
- Mental focus and mood
- Skin moisturizing (cocoa butter)
Cocoa is rich in plant compounds called flavanols that may modestly support blood vessel function and blood pressure, but most chocolate products are high in sugar, fat, and calories, which can offset any benefit. Enjoy cocoa as part of a balanced diet, and talk to your pharmacist before using concentrated cocoa supplements, especially if you have heart or blood pressure conditions.
