Here's the thing to know about pairing cocoa with finafloxacin (Xtoro): it isn't really the antibiotic that builds up, it's the caffeine in the cocoa. Fluoroquinolone antibiotics like finafloxacin can slow down how fast your body clears caffeine, so the caffeine you take in tends to hang around longer and hit a little harder.

In practice that could mean feeling jittery, a racing heart, trouble sleeping, or an upset stomach. The good news is that finafloxacin you'd be asking about is usually Xtoro ear drops, which barely get into the bloodstream, so a cup of hot cocoa is unlikely to cause much. The evidence here is modest and mostly theoretical. Still, if you're sensitive to caffeine, mention it to your pharmacist.
