Fluticasone, Salmeterol falls under BETA-ADRENERGIC AGONISTS — the group Cocoa interacts with.
Cocoa contains caffeine. Theoretically, large amounts of caffeine might increase cardiac inotropic effects of beta-agonists. A case of atrial fibrillation associated with consumption of large quantities of chocolate in a patient with chronic albuterol inhalation abuse has also been reported.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Advair pairs two medicines: fluticasone (an inhaled steroid) and salmeterol, a long-acting beta-agonist that opens your airways. The piece that matters here is salmeterol. Beta-agonists can nudge your heart to beat a little faster and harder, and the caffeine-like compounds in cocoa may add to that effect. So in theory, washing down your puffer with a lot of chocolate or a strong cocoa supplement could leave you feeling jittery, with a racing or pounding heartbeat.
Honestly, this is a theoretical concern based mostly on how these things work, plus one unusual case. A normal cup of hot cocoa or a square of chocolate is very unlikely to cause trouble. Concentrated cocoa supplements are where I'd be more cautious, especially if you have heart rhythm issues. Check with your pharmacist before adding one.
