Here's the deal with cocoa and phenelzine (Nardil). Phenelzine is an MAOI, which means it blocks an enzyme your body uses to break down certain stimulating substances. Cocoa naturally contains caffeine, so in theory, taking in a lot of it at once could let caffeine's effects pile up more than usual. That could nudge your blood pressure up and possibly trigger a hypertensive crisis (a dangerous spike in pressure), with symptoms like a pounding headache, a racing heart, sweating, or chest discomfort.

The good news: this is a theoretical concern, and a normal piece of chocolate or a cup of cocoa is unlikely to cause trouble. The bigger worry would be concentrated cocoa or caffeine supplements. Please run any cocoa supplement past your pharmacist or doctor first, since MAOIs already come with strict food and blood pressure precautions.

What Is Phenelzine Used For?

Brand name: Nardil

Phenelzine is used to treat depression — specifically the types described as atypical, nonendogenous, or neurotic depression. This often includes people who have a mix of anxiety and depression, or who have phobic or hypochondriacal features (strong fears about illness or physical symptoms).

It is generally not the first antidepressant a doctor will try. It is best suited for patients who have already tried other antidepressants without enough relief. There is less evidence supporting its use in severe depression with classic biological features (called endogenous depression).
