Here's the thing about coffee charcoal: it acts like a sponge in your gut. That porous, charred surface can grab onto things you swallow, including ferric citrate (Auryxia), and keep your body from fully absorbing it. This isn't a dramatic chemical reaction, it's more of a physical binding issue, and it's based on how charcoal behaves rather than hard human studies.
If less of your ferric citrate gets absorbed, it may not control your phosphorus as well, or it may treat your anemia less effectively. To stay safe, just space them out. Taking your ferric citrate at least two hours apart from the coffee charcoal usually solves this. Before mixing the two, run it by your pharmacist or kidney doctor so your levels stay on track.

Ferric citrate has two uses in adults with chronic kidney disease. Which one applies depends on whether you are on dialysis.
- On dialysis: it is a phosphate binder. It controls the level of phosphorus in the blood (hyperphosphatemia).
- Not on dialysis: it is an iron replacement product. It treats iron deficiency anemia (low iron causing low red blood cell function).
Both Auryxia and ferric citrate tablets carry these same two uses.
