Here's the gist with Demerol (meperidine) and coffee charcoal. Charcoal is porous and acts a bit like a sponge in your gut. If you swallow it close to your oral meperidine dose, it may grab onto the medicine and keep some of it from being absorbed into your bloodstream. The likely result is less pain relief, not more, since you'd be soaking up part of the dose before it can work.

To be clear, this is a theoretical concern based on how charcoal behaves, not strong human studies. The simple fix is timing: keep coffee charcoal and your meperidine at least two hours apart. If your pain control feels off, please loop in your pharmacist or doctor before changing anything.
