Here's the simple version: coffee charcoal works like a sponge in your gut. Its porous, charred surface can grab onto things passing through your digestive tract, and that includes medicine you swallow. If you take your oral pentazocine (Talwin) too close to a dose of coffee charcoal, some of the drug may get bound up and never fully absorbed into your bloodstream.
For pain relief, that usually means weaker effect, so your pain may not be controlled as well as expected. This is a theory based on how charcoal behaves, not a proven, measured interaction, so don't panic. The easy fix is timing: separate the two by at least two hours. Check with your pharmacist or doctor before combining them so your pain stays managed.
