Here's the gist with pramipexole (Mirapex) and coffee charcoal. Like other charcoal-type products, this supplement has a porous surface that can grab onto medicines sitting in your stomach and intestines. If they're in there at the same time, less of your pramipexole gets absorbed into your body, which could mean a weaker effect. For Parkinson's, that might look like more shaking, stiffness, or slowed movement creeping back; for restless legs, your symptoms could return at night.
This is a theoretical concern based on how charcoal behaves, not strong human studies, so don't panic. A simple fix is spacing them out by at least two hours. Before you combine them, run it by your pharmacist or doctor first.

Pramipexole tablets (immediate-release) are approved to treat Parkinson's disease and moderate-to-severe primary restless legs syndrome (RLS). Pramipexole dihydrochloride extended-release tablets are approved only for Parkinson's disease.
For Parkinson's disease, pramipexole helps manage the motor symptoms of the condition. For RLS, the immediate-release tablet is taken to relieve the uncomfortable urge-to-move sensations that are the hallmark of the syndrome.
