Here's the simple version: coffee charcoal acts like a sponge in your gut. Its porous, charcoal-like surface can grab onto things you swallow, including medicines, and that can keep some of the drug from being absorbed the way it should. If you take it right alongside your simethicone, the charcoal could soak up some of the simethicone, so you may get less gas relief and still feel that bloating, fullness, or pressure.

The good news is that simethicone works right in the stomach and intestines and isn't a high-stakes medication, so this isn't dangerous, just a possible loss of effect. To be safe, separate them by at least two hours. This warning is based on how charcoal behaves in theory, not hard studies, so check with your pharmacist if you take both regularly.

Simethicone relieves the uncomfortable pressure, bloating, and full or stuffed feeling that comes from trapped gas. Gas can build up after eating certain foods or from swallowing air.

Infant-specific products — including Mylicon Infants Gas Relief, Walgreens Infants Dye Free Gas Relief, and similar infant drops and suspensions — are used to relieve gas discomfort in babies frequently caused by air swallowing or certain formulas or foods.
