Here's the gist for you. Spironolactone is a water pill that helps your body shed extra fluid and lower your blood pressure. The concern with coltsfoot is fairly mild and mostly theoretical. The worry is that large, concentrated amounts of coltsfoot might nudge your blood pressure in a way that works against what your spironolactone is doing, so your numbers could be less steady than you'd like.

To be honest, this is based on weak, anecdotal evidence, not solid human studies. The bigger reason I'd steer clear of coltsfoot is its naturally occurring compounds that can be hard on the liver, which matters even more if you take spironolactone for liver-related swelling. Please check with your pharmacist or doctor before adding it.

Coltsfoot is a traditional herb long used to soothe coughs and clear mucus, but solid human evidence for these uses is lacking. More importantly, the raw plant can contain liver-toxic pyrrolizidine alkaloids, so it carries real safety concerns and is best avoided unless you use a product certified to be free of these compounds.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
