Fruquintinib falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with pairing comfrey and Fruquintinib (Fruzaqla), your colon cancer pill. Fruquintinib can already be tough on the liver, and your care team likely checks your liver blood tests while you're on it. Comfrey contains natural compounds called pyrrolizidine alkaloids that can also harm the liver, especially when swallowed. Put the two together and you could, in theory, stack up the liver stress.

To be fair, this is a theoretical caution based on how each one works, not proven cases with this exact drug. Still, since comfrey should never be taken by mouth anyway, I'd steer clear of oral comfrey while on Fruquintinib. If you're only using it briefly on unbroken skin, mention it to your oncologist or pharmacist first so they can weigh in.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. Pregnant or breastfeeding people should avoid it entirely.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
