Naltrexone Hydrochloride falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with pairing these two. Comfrey contains natural compounds called pyrrolizidine alkaloids, and when taken by mouth they can be quite hard on the liver. Naltrexone can also stress the liver, especially at higher doses, which is why your doctor may check your liver numbers now and then. Putting both together could theoretically add up to more strain on your liver than either one alone.

I want to be honest that this is a theoretical caution based on how these substances behave, not proof it will happen to you. Still, comfrey really shouldn't be swallowed at all because of its liver risks. Watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before combining them.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. Pregnant or breastfeeding people should avoid it entirely.

Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people with opioid dependence. The extended-release injectable suspension — sold as Vivitrol and also available as a generic injectable — is used to treat alcohol dependence and to prevent relapse to opioid dependence after a person has completed opioid detoxification.

All formulations are meant to be part of a broader treatment plan that includes counseling and other support — naltrexone works best when it is not used alone.
