Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with pairing comfrey and sulfamethoxazole (Gantanol). Comfrey contains natural compounds called pyrrolizidine alkaloids that can be hard on the liver, which is why it should never be swallowed. Sulfamethoxazole, the antibiotic, also carries its own small risk of liver irritation in some people. Put the two together and you're potentially stacking two stressors on the same organ.

To be honest with you, this is a theoretical caution based on how each one works, not something proven by strong studies. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Topical comfrey on intact skin for a few days is a different story, but please check with your pharmacist or doctor before combining them.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses.
