Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with pairing comfrey and ritonavir. This isn't about comfrey changing how ritonavir works in your body. It's about both being tough on the liver. Ritonavir can already raise liver enzymes and, in some people, cause more serious liver trouble. Comfrey taken by mouth contains pyrrolizidine alkaloids that are known to damage the liver. Stacking the two could add up and put more strain on that organ.

I'll be honest, this is a theoretical caution based on how each one behaves, not on real cases of people combining them. Still, comfrey should never be swallowed by anyone. If you're on ritonavir and curious about comfrey, please check with your pharmacist or HIV provider first.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. Pregnant or breastfeeding people should avoid it entirely.

What Is Ritonavir Used For?

Brand name: Norvir

Ritonavir (brand name Norvir) is used together with other HIV medicines to treat HIV-1 infection in adults and children older than one month of age. It is always taken as part of a combination regimen — never on its own.

Norvir oral powder is specifically indicated for pediatric patients with HIV-1 infection. Norvir tablets and oral solution are indicated for both adults and pediatric patients with HIV-1 infection. Ritonavir tablets (generic) are also indicated for the treatment of HIV-1 infection in combination with other antiretroviral agents.
