Teclistamab-cqyv falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the simple version. Teclistamab (Tecvayli) is a powerful antibody therapy for multiple myeloma, and it can stress your liver on its own, so your care team already watches your liver labs closely. Comfrey is the concern here. When taken by mouth, it contains pyrrolizidine alkaloids that can genuinely harm the liver, sometimes seriously. Stacking two things that both burden the liver is just adding risk you don't need.

This concern is theoretical, based on how these substances behave rather than reported cases together, but the liver risk from oral comfrey is real and well known. Please never swallow comfrey, and check with your oncology team or pharmacist before using even topical comfrey while on Tecvayli.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. Pregnant or breastfeeding people should avoid it entirely.

What Is Teclistamab-cqyv Injection Used For?

Brand name: Tecvayli

Tecvayli (teclistamab-cqyv) injection is used to treat adults with relapsed or refractory multiple myeloma — a cancer of plasma cells in the bone marrow — whose disease has come back or stopped responding to earlier treatments.

Tecvayli is approved in two settings: as monotherapy for patients who have received at least four prior lines of therapy (including a proteasome inhibitor, an immunomodulatory agent, and an anti-CD38 monoclonal antibody); and in combination with daratumumab and hyaluronidase-fihj for patients who have received at least one prior line of therapy including a proteasome inhibitor and an immunomodulatory agent.
