Here's the simple version: Clarithromycin (Biaxin) is a strong blocker of an enzyme called CYP3A4, and your body uses that same enzyme to help break down THC. When the enzyme is slowed down, THC can clear out of your system more slowly and build up a bit. In plain terms, the cannabis effects could feel stronger or last longer, so think more drowsiness, dizziness, a foggier head, or a more intense high than you'd usually get.

I'll be honest, this one is theoretical. It's based on how these two work together, not on solid studies showing problems in real people. Still, if you take clarithromycin while using THC, go easy and don't drive until you know how you feel. It's always worth a quick chat with your pharmacist or doctor.

Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist for clarithromycin. Your liver uses an enzyme called CYP3A4 to help clear this antibiotic, and lab studies suggest THC might slow that enzyme down a little. In theory, that could let clarithromycin build up slightly, which might mean a bit more of its usual side effects, like an upset stomach, a strange or metallic taste, or loose stools.

That said, this is really weak evidence. It comes from test-tube work, and a small human study using oral THC actually showed no effect on CYP3A4. So a meaningful problem here is unlikely. Still, since both clarithromycin and THC can affect how you think and feel, it's worth a quick chat with your pharmacist or doctor if you use cannabis regularly.

A prescription form (dronabinol) is approved for chemotherapy-related nausea and appetite loss, but for most other uses the evidence is mixed or limited, and THC can impair thinking and driving. It should be avoided in pregnancy and breastfeeding and used only with medical guidance.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
