The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the gist with Dendrobium and pyrimethamine (Daraprim). One natural compound in Dendrobium called dendrobine has been reported to act as a convulsant, meaning it may make the brain a little more prone to seizures. Pyrimethamine is grouped with medicines that can lower the seizure threshold, so the worry is that stacking the two together could, in theory, nudge up the risk of a seizure.

I want to be honest about how strong this is: it's a theoretical concern based on how these compounds behave, not something proven in people. So no need to panic. Still, since seizures are serious, it's worth a quick chat with your pharmacist or doctor before combining them rather than starting Dendrobium on your own.
