Eleuthero + Netupitant, Palonosetron (Oral)
Netupitant, Palonosetron (Oral) brand name: Akynzeo.
Eleuthero has a minor interaction with Netupitant, Palonosetron (Oral). Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Netupitant, Palonosetron (Oral)
Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for Akynzeo (netupitant plus palonosetron). The netupitant part is cleared by a liver enzyme called CYP3A4. In test-tube and animal studies, eleuthero looked like it might slow that enzyme down a little, which in theory could let netupitant build up and lead to more of its usual side effects, like headache, constipation, tiredness, or weakness.
The good news is that when researchers actually tested eleuthero in healthy people, it did not meaningfully block CYP3A4. So this is mostly a paper concern, rated minor and unlikely. Still, since Akynzeo is part of your chemo care, it's worth a quick mention to your oncology team or pharmacist before adding eleuthero.

From our pharmacist’s overview on the Netupitant, Palonosetron (Oral) page
“Getting Akynzeo (netupitant and palonosetron) to work is the difference between finishing a chemotherapy cycle comfortably and losing days to nausea, so this list warrants genuine care. Most of it involves a liver enzyme, CYP3A4, that clears netupitant.”

About Eleuthero
What is Eleuthero?
- Fatigue and low energy
- Physical and athletic performance
- Stress resistance (adaptogen)
- Immune support
- Mental focus and alertness
Eleuthero is an herb traditionally used as an 'adaptogen' to fight fatigue, boost energy, and help the body handle stress. The scientific evidence behind these uses is limited and mixed, so it should not replace proven treatments. It is generally well tolerated short-term, but pregnant and breastfeeding people and those with certain conditions should avoid it or check with a healthcare provider first.
About Netupitant and Palonosetron
What Is Netupitant and Palonosetron Used For?
Brand name: Akynzeo
Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).
Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
