Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group European Barberry interacts with.

Theoretically, European barberry might increase the levels and clinical effects of CYP3A4 substrates.
There is very preliminary evidence suggesting that berberine, a constituent of European barberry, might inhibit the CYP3A4 enzyme. Theoretically, European barberry might have a similar effect.

Here's the gist of it. Your body uses a liver enzyme called CYP3A4 to help break down and clear selpercatinib (Retevmo). Berberine, the main active compound in European barberry, may slow that enzyme down. If it does, your selpercatinib could clear more slowly and build up in your blood, which can mean stronger effects and a higher chance of side effects like high blood pressure, liver enzyme changes, tiredness, swelling, diarrhea, or heart rhythm changes (a longer QT interval).

I want to be straight with you though: this is a theoretical concern based on lab pharmacology, not real-world reports. Still, because selpercatinib is a serious cancer medicine, it's not worth the gamble. Please check with your oncologist or pharmacist before adding barberry or berberine.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
