Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's something worth knowing about Apadaz. The benzhydrocodone part is actually a prodrug, meaning your body has to convert it into the active painkiller (hydrocodone) before it does much. The enzyme CYP2D6 then further processes hydrocodone into an even stronger compound. False Unicorn has shown, in lab testing only, that it may block CYP2D6.

Because of that, the worry runs in two directions and isn't fully clear: blocking this enzyme could let hydrocodone build up (more drowsiness, slowed breathing, constipation, nausea) or it could blunt some pain relief. Honestly, this is only a theoretical concern from a test-tube study, so I wouldn't panic. Still, with an opioid involved, please run it by your pharmacist or doctor first.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's a wrinkle worth knowing: the benzhydrocodone in Apadaz is a prodrug. Your body has to convert it into hydrocodone before it actually relieves pain. In lab studies, False Unicorn appears to slow down an enzyme called CYP3A4, which is one of the pathways involved in handling hydrocodone once it's formed. If that enzyme is blocked, the active opioid could linger and build up rather than clear out normally.

In plain terms, that could mean stronger or longer opioid effects, things like extra drowsiness, dizziness, constipation, or slowed breathing. I'll stress this is theoretical and based only on test-tube data, so we don't truly know how much it matters in real life. Still, with an opioid, I'd play it safe. Please check with your pharmacist or doctor before combining the two.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
