Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for Rexulti (brexpiprazole). Your body relies on a liver enzyme called CYP2D6 to help break this medication down and clear it out. In lab testing (not in actual people), False Unicorn appears to slow that enzyme down. If that happened in your body, brexpiprazole could build up a bit higher than usual, which might mean more side effects like drowsiness, dizziness, restlessness, weight gain, or feeling unsteady.

I want to be honest with you though: this is only based on test-tube data, so it's theoretical and far from proven. Still, since False Unicorn hasn't been well studied for safety, please check with your pharmacist or doctor before combining the two rather than starting or stopping anything on your own.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to help break down brexpiprazole (Rexulti). In a lab dish, False Unicorn appears to slow that same enzyme down. If that happened in real life, brexpiprazole could clear out more slowly and build up a bit higher than usual.

Brexpiprazole is an active drug, not a prodrug, so higher levels would mean stronger effects and possibly more side effects, things like drowsiness, dizziness, restlessness (feeling like you can't sit still), or weight changes. I want to be honest, though: this is only a test-tube finding, not proof it happens in people, so don't panic. Still, since this medicine matters for your mental health, please run the combination by your pharmacist or doctor before mixing the two.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
