Dextromethorphan, Methscopolamine, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

In this combo product, the part to focus on is the dextromethorphan (the cough suppressant). Your liver uses an enzyme called CYP2D6 to clear it out. False Unicorn, at least in lab dish studies, seems to slow that same enzyme down. If that held true in real life, dextromethorphan could build up a bit and lead to stronger side effects like dizziness, drowsiness, or feeling foggy. Dextromethorphan is not a prodrug, so blocking CYP2D6 means more drug effect, not less.

Here's the honest part: this is only a test-tube finding, not something proven in people, so I wouldn't panic. Still, since safety data on False Unicorn is thin, run it by your pharmacist or doctor before pairing them.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

This combination cold-and-cough product contains dextromethorphan (for cough), which your liver partly clears using an enzyme called CYP3A4. In a lab dish, False Unicorn appeared to slow that enzyme down. In theory, that could let dextromethorphan build up a bit more than usual, which might mean extra drowsiness, dizziness, or feeling foggy. The methscopolamine and phenylephrine pieces aren't really CYP3A4-driven, so this concern centers on the dextromethorphan.

Here's the honest part: this is based only on test-tube data, not real people, so the actual risk is uncertain and probably small. Still, since False Unicorn hasn't been well studied for safety, I'd run it by your pharmacist or doctor before pairing the two.
