In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the piece that matters in your combination product: the cough suppressant dextromethorphan is cleared by a liver enzyme called CYP2D6. In a lab dish, False Unicorn appears to slow that enzyme down. If that happened in your body, dextromethorphan could build up a bit more than usual, which might mean more drowsiness, dizziness, or feeling foggy. The pseudoephedrine and triprolidine pieces aren't the main concern here.

I want to be straight with you, though: this is only a test-tube finding, not something proven in real people, so the actual risk is likely small. Still, since you're combining a few medicines already, it's worth a quick check with your pharmacist or doctor before adding False Unicorn.

Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist with Actifed DM (a combo of dextromethorphan for cough, pseudoephedrine as a decongestant, and triprolidine as an antihistamine). In a lab dish, False Unicorn appears to slow down an enzyme called CYP3A4, which helps your body break down the dextromethorphan piece. If that enzyme runs slower, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy.

I want to be honest about the strength of this, though. It's only a test-tube finding, not something proven in real people, so it may not amount to much. Still, since both products can make you sleepy on their own, it's worth a quick chat with your pharmacist before combining them.
