Here's the key thing about Actifed DM: one of its ingredients is pseudoephedrine, which is itself a stimulant very similar to the ephedrine in ephedra. So you'd basically be stacking two stimulants on top of each other. That can push your heart rate and blood pressure higher than either one alone, leading to a pounding or racing heart, jitteriness, headache, trouble sleeping, and in serious cases dangerous spikes in blood pressure.

This concern is rated as major, though the evidence is mostly anecdotal and partly theoretical. Still, ephedra is banned in U.S. supplements for good reason, and I wouldn't pair it with anything containing pseudoephedrine. Please check with your pharmacist or doctor about safer cold and cough options.
