Yohimbine, a constituent of Rauvolfia vomitoria, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking Rauvolfia vomitoria with stimulant drugs can have additive stimulant and hypertensive effects.

The piece of this combo product I'd watch is the pseudoephedrine, which is a decongestant that gently revs up the body the same way adrenaline does. It can raise blood pressure and heart rate on its own. Rauvolfia vomitoria contains yohimbine-type compounds that also stimulate the body and can push blood pressure up. Stacking the two together could, in theory, add up to higher blood pressure, a racing or pounding heart, jitteriness, headache, or trouble sleeping.

To be fair, this concern comes from how these compounds work, not from strong studies in people, so it's more of a caution than a proven event. Still, since both can affect your blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist with Actifed DM: one of its ingredients, dextromethorphan (the cough suppressant), is cleared from your body by a liver enzyme called CYP2D6. Rauvolfia vomitoria carries a small amount of yohimbine, and in lab studies yohimbine can slow that enzyme down. In theory, that means dextromethorphan could build up a bit higher than usual, which might bring on more drowsiness, dizziness, or feeling foggy and out of it.

I'll be honest, this is based on test-tube research only, so it's more of a caution than a proven problem. The bigger concern is actually the blood pressure angle, since both rauvolfia and the pseudoephedrine in this product affect blood pressure in opposite ways. Please check with your pharmacist or doctor before combining these.
