Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the connection worth knowing. Padcev is an antibody-drug combination, and once it gets inside cancer cells it releases a chemotherapy payload called MMAE that your body clears partly through an enzyme named CYP3A4. In lab tests (test tube only, not people), False Unicorn appears to slow that same enzyme down. In theory, blocking CYP3A4 could let more of that toxic payload linger, which might raise the chance of Padcev's known side effects like numbness or tingling in the hands and feet, skin rash, fatigue, nausea, or higher blood sugar.

I want to keep this in perspective. This is theoretical and based only on early lab data, not real-world reports. Still, because this is serious cancer treatment, please clear any herbal product with your oncology team first.

What Is Enfortumab vedotin-ejfv Injection Used For?

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
